| Patient-Centered Counseling DVD | A DVD/CD product that teaches providers how to counsel individual patients/clients at any stage of readiness that the patient is at in order to help them make healthier behavior changes. This training supports prevention counseling during the Periodic Health Assessment or during any other primary care or health education encounter on any health topic that is related to behavior change. | Prevention First Self-help CD | The purpose of the PRxEVENTION FIRxST Self-help CD is to reduce or prevent health risks and conditions such as inactivity, obesity, diabetes, hyperlidemia (elevated blood lipids), and hypertension (high blood pressure), thus delaying or preventing individuals from needing medications for these conditions. The PRxEVENTION FIRxST Self-help CD provides easy access to tailored exercise, nutrition, and behavior change programs for all at risk adult TRICARE beneficiaries. Multiple orders are limited to 25 per order. |
