Background
Population Health Navigator (PHN) is a tool for medical informatics selected by Bureau of Medicine (BUMED) to assist the Medical Treatment Facilities (MTFs) in actively managing their patient populations and is the means by which the BUMED metrics are calculated (BUMED Note 6310). PHN offers aggregate metrics and lists with actionable data for disease management by MTF.
